RESUMO

Aging may increase the risk of maxillofacial and dentoalveolar trauma (MDT). This review assessed global trends in scientific publications on MDT in the elderly. Six databases were searched. Studies about MDT in the elderly (≥60 years old) were included in two text mining stages (S1 and S2). In S1, all studies with elderly subjects were included, while in S2 only studies in which the elderly were assessed in a separate set of data or in a subanalysis by age groups were included. In S1, relevant data were mined using VantagePoint™ software. In S2, data were descriptively analyzed. There were 2265 studies at S1 and 110 at S2. The publication dates spanned from 1963 to 2023. In S1, the most cited keywords and terms were aged (n = 1872), male (n = 1839), female (n = 1777), "mandible fracture" (n = 460), "orbit fracture" (n = 362), and "maxilla fracture" (n = 118). Thirty authors published 10 or more articles with visible collaboration networks. There was a progressive increase in publications, especially in Europe (n = 150). In S2, most studies were case reports/case series (n = 62). The primary etiology was accidental falls (n = 84), the most affected region was the middle third of the face (n = 65), and dental trauma was rarely reported (n = 15). Cone Beam computed tomography was the most used diagnostic tool (n = 67). Of the 81 articles that addressed therapy, 52 opted for surgical treatment. Although there has been an increase in the number of articles on MDT over the years, studies with the elderly population as a specific study group are still needed. Accidental falls were the most reported etiology, while the middle third of the face and the mandible were the anatomic region and bone most affected, respectively. The high number of case reports indicates the need for improvement in the available scientific evidence about MDT in the elderly.

Although resin composites are widely used in the clinical practice, the development of recurrent caries at composite-tooth interface still remains as one of the principal shortcomings to be overcome in this field. Objectives To evaluate the activity against S. mutans biofilm of model resin composites incorporating different concentrations of ZnO-nanoparticles (ZnO-NP) and characterize their physicochemical properties. Materials and Methods Different concentrations of ZnO-NP (wt.%): E1=0, E2=0.5, E3=1, E4=2, E5=5 and E6=10 were incorporated into a model resin composite consisting of Bis-GMA-TEGDMA and barium borosilicate particles. The activity against S. mutans biofilm was evaluated by metabolic activity and lactic acid production. The following physicochemical properties were characterized: degree of conversion (DC%), flexural strength (FS), elastic modulus (EM), hardness (KHN), water sorption (Wsp), water solubility (Wsl) and translucency (TP). Results E3, E4, E5 and E6 decreased the biofilm metabolic activity and E5 and E6 decreased the lactic acid production (p<0.05). E6 presented the lowest DC% (p<0.05). No significant difference in FS and EM was found for all resin composites (p>0.05). E5 and E6 presented the lowest values of KHN (p<0.05). E6 presented a higher Wsp than E1 (p<0.05) and the highest Wsl (p<0.05). The translucency significantly decreased as the ZnO- NP concentration increased (p<0.05). Conclusions The incorporation of 2 - 5 wt.% of ZnO-NP could endow antibacterial activity to resin composites, without jeopardizing their physicochemical properties.

The presence of fixed orthodontic appliances and its accessories tends to affect bacterial plaque removal, resulting in inadequate cleaning being performed by the patient. Thus, the aim of this systematic review and meta-analysis was to answer the following focal question: are the mechanical methods of oral cleaning effective for the reduction of bacterial dental plaque in patients with a fixed orthodontic appliance? A systematic search of the literature was conducted in the following databases: Scopus, PubMed, Cochrane, Web of Science, Embase, and Medline, in addition to Controlled Trials and Grey Literature, without limitations of language and year of publication. Furthermore, searches were also conducted in the reference lists of the articles selected. Included in the present review were clinical studies with patients undergoing therapy with fixed orthodontic appliances, which would, by means of comparison, evaluate the effectiveness of different mechanical bacterial plaque removal methods. The methodological quality of the articles included was evaluated in accordance with the methodological evaluation protocol, and was classified as high, moderate or low. In order to perform the meta-analysis, articles with high methodological quality were included. Analysis of subgroups was performed according to the type of intervention. The plaque index was defined as the continuous outcome variable, using the fixed effect when there was homogeneity among the articles, and the random effect in cases of heterogeneity. 1815 articles were found, of which only 23 fulfilled the eligibility criteria, with 17 being of high methodological quality and 6 of moderate quality. Of these, 18 included an analysis of the effectiveness of the conventional manual brush, 4 of the manual orthodontic brush,15 of electric brushes, 3 of interdental brushes, 1 of dental floss and 4 or oral irrigation devices, and 1 of Siwak. In the qualitative evaluation of the studies, the use of these mechanical control methods was shown to be effective, with exception of the use of Siwak, which presented an increase in bacterial plaque. Moreover, the results of the meta-analysis indicated a significant reduction of -0.98 (IC 95%) in the plaque index for the use of conventional manual brushes in the period of one month (p<0.001). There is a body of evidence that proves the effectiveness of mechanical bacterial plaque removal in patients using orthodontic appliances and accessories. The findings of the meta-analysis proved the effectiveness of the conventional manual brush. Further studies with better standardization of samples are required to strengthen the evidences.


A presença de aparelhos ortodônticos fixos e seus acessórios tende a afetar a remoção da placa bacteriana, resultando em limpeza inadequada realizada pelo paciente. Assim, o objetivo desta revisão sistemática e metaanálise foi responder à seguinte questão norteadora: os métodos mecânicos de limpeza oral são eficazes para a redução da placa bacteriana dental em pacientes com aparelho ortodôntico fixo? Uma pesquisa sistemática da literatura foi realizada nas seguintes bases de dados: Scopus, PubMed, Cochrane, Web of Science, Embase e Medline, além de Ensaios Controlados e Literatura Cinzenta, sem limitações de idioma e ano de publicação. Além disso, as pesquisas também foram realizadas nas listas de referência dos artigos selecionados. Foram incluídos na presente revisão estudos clínicos realizados com pacientes submetidos à terapia com aparelhos ortodônticos fixos, os quais, por meio de comparação, avaliaram a eficácia de diferentes métodos mecânicos de remoção de placas bacterianas. A qualidade metodológica dos artigos incluídos foi avaliada de acordo com o protocolo de avaliação metodológica, e classificada como alta, moderada ou baixa. Para realização da metanálise, foram incluídos artigos de alta qualidade metodológica. A análise dos subgrupos foi realizada de acordo com o tipo de intervenção. O índice de placa foi definido como variável de desfecho contínuo, utilizando o efeito fixo quando houve homogeneidade entre os artigos e o efeito aleatório nos casos de heterogeneidade. 1815 artigos foram encontrados, dos quais apenas 23 preencheram os critérios de elegibilidade, sendo 17 de alta qualidade metodológica e 6 de qualidade moderada. Destes, 18 incluíram uma análise da eficácia da escova manual convencional, 4 da escova ortodôntica manual, 15 de escovas elétricas, 3 de escovas interdentais, 1 de fio dental e 4 de dispositivos de irrigação oral e 1 de Siwak. Na avaliação qualitativa dos estudos, a utilização destes métodos mecânicos de controle mostrou-se efetiva, com exceção do uso de Siwak, que apresentou aumento na placa bacteriana. Além disso, os resultados da meta-análise indicaram uma redução significativa de -0,98 (IC 95%) no índice de placa para o uso de escovas manuais convencionais no período de um mês (p <0,001). Existe um conjunto de provas que comprovam a eficácia da remoção de placas bacterianas mecânicas em pacientes que utilizam aparelhos e acessórios ortodônticos. Os resultados da metanálise provaram a eficácia da escova manual convencional. Outros estudos com melhor padronização de amostras são necessários para fortalecer as evidências.

BACKGROUND: The aim of this systematic review was to determine whether there is any evidence in the literature referring to a lower prevalence of dental caries in children and adolescents with chronic kidney disease (CKD) compared to healthy individuals. METHODS: A search of the PubMed Medline, Ovid Medline and Cochrane Library databases was performed using the MeSH terms "dental caries" and "chronic renal failure". To be eligible for entry in our study, controlled observational studies had to present a decayed, missing and filled index for primary teeth (dmft) and/or for permanent teeth (DMFT) in children and adolescents with CKD. RESULTS: After evaluation of title, keywords and abstracts of the articles selected, six articles met the inclusion criteria. Three of these six articles included studies which showed susceptibility to bias and possible confounding factors. A subsequent assessment of the six studies revealed that the mean caries indices in both primary (dmf) and permanent (DMF) teeth were lower in the children and adolescents with CKD compared with healthy individuals. CONCLUSION: Data in the literature weakly support a lower prevalence of caries in children and adolescents with CKD than in their healthy counterparts. There is still a lack of well-designed studies that provide better scientific evidence in support of this conclusion.

PURPOSE: To evaluate the influence of four surface treatments on the bond strength of a self-adhesive resin cement to an yttria-stabilized zirconia (Y-TZP) ceramic material (Lava Frame zirconia). MATERIALS AND METHODS: Forty plates (8 x 6 x 1 mm) of a Y-TZP ceramic restorative material were randomly assigned to four groups (n = 10) according to the surface treatments: control, no treatment; airborne-particle abrasion with 50-µm Al2O3; coating with an MDP-based primer; conditioning with Rocatec System. The ceramic plates treated with each of the four methods were further divided into 2 subgroups according to the resin cement tested: RelyXTM ARC (ARC, conventional) and RelyXTM Unicem (Ucem, self-adhesive). The resin cements were put into PVC tubes (diameter 0.75 mm, 0.5 mm height) placed on the ceramic plate surfaces. After water storage at 37°C for 24 h, the specimens were submitted to a microshear bond strength (µSBS) test at a crosshead speed of 1.0 mm/min. RESULTS: The surface treatments significantly influenced the µSBS (p < 0.05). For the four surface treatments, UCem presented significantly higher µSBS than ARC (p < 0.05). For both resin cements, the best result was produced by the MDP-based primer: ARC 15.9 ± 5.0 MPa and UCem 36.2 ± 2.1 MPa. The highest µSBS values were presented by UCem on ceramic plates treated with the MDP-based primer (36.2 ± 2.1 MPa) and Rocatec system (37.4 ± 2.3 MPa). CONCLUSION: Irrespective of the surface treatment, the self-adhesive resin cement performed better in terms of bond strength to yttria-stabilized zirconia ceramic than did conventional resin cement.

O presente estudo piloto teve por objetivo determinar o grau de exposição à sacarose de pacientes infantis atendidos na clínica de Odontopediatria da Unversidade Salgado de Oliveira - UNIVERSO/SP, do qual participaram 15 pacientes, na faixa etária entre 02 e 12 anosde idade, de ambos os sexos. Um inquérito de diete foi preenchido, voluntariamente, pelos responsáveis pelas crianças, a fim de calcular o índice de exposição à sacarose, tomando-se como referência o momento de ingestão e a consistência do alimento. Pode-se verificar uma ampla variação nos índices individuais de exposição à sacarose, representado por um valor mínimo de 2,3 e máximo de de 13,3 e média de 8,0 +- 3,9. O desequilíbrio na exposição à sacarose encontrado no presente estudo aponta a necessidade de um maior aprofundamento dos resultados obtidos, a fim de se estabelecer correlações entre os valores do índice de exposição à sacarose encontrados e a prevalência cárie nas crianças, bem como para se compreender melhor o papel dos hábitos de higiene, utilização de fluoretos, e processo educativo em saúde bucal, na prevenção e controle da doença na população infantil

RESUMO

O granuloma piogênico representa uma hiperplasia inflamatória assintomática da pele ou da mucosa e pode causar interpretações errôneas devido ao seu rápido crescimento, sendo confundido com processos malignos. São apresentados, neste trabalho, dois casos de granuloma piogênico em crianças, na fase de dentição decídua, atendidas na Clínica de Odontopediatria da Universidade do Estado do Rio de Janeiro. O tratamento consistiu na remoção total do tecido de granulação. O controle pós-operatório não revelou sinais de recidiva
